What you need to provide
1 Issue details

You need to:

  • provide information about the issue (e.g. the type of issue you're reporting, if traffic is swerving to avoid the hazard, etc.)
  • let us know if the issue is a public safety concern
  • upload photos (optional).
2 Location information

Provide the location of the issue, including the street, suburb and postcode. 

3 Contact details (optional)

You can provide contact details, such as a name, phone number and email address.
